Here are your card details. To replace any of these cards simply select lost, stolen or damaged from the dropdown list next to the relevant card and press 'submit'. We will then do the rest for you.

What happens next?

You will receive your new card in 1-2 weeks. In the meantime, if you are shopping in store, ask the cashier to validate your receipt so a member of staff can add your points to your account when your new card arrives. You can carry on shopping on Boots.com as normal. All points collected will be added to your account. We will also update the Advantage Card number on your Boots.com account

Need a new card following a name change?

If you?ve changed your name and would like to request a new card, please change your name on the 'My details' page and we will send a new card to you.
